Hypothyroidism occurs when the thyroid gland fails to produce enough thyroid hormone.As a result the body becomes tired. Every organ system slows, including the brain, which affects concentration; the gut, causing constipation; and metabolism – the rate at which the body burns energy – resulting in weight gain.
The most common cause of hypothyroidism is Hashimoto's thyroiditis, in which the body's immune system attacks the thyroid gland. Other causes include congenital defects, surgical removal of the thyroid gland, irradiation of the gland and inflammatory conditions. The condition is more common in women and people over the age of 50. Other risk factors include thyroid surgery and exposure of the neck to X-ray or radiation treatments.
Hyperthyroidism occurs when the thyroid produces excess thyroid hormone. The most common cause of the condition is Graves' disease, which accounts for 85 percent of cases. Hyperthyroidism also can result from nodular goiter, or condition in which an inflammation of the thyroid occurs due to viral infections or other causes.

A goitre is an enlargement of the thyroid gland. A goitre can be smooth and uniformly enlarged, called diffuse goitre, or it can be caused by one or more nodules within the gland, called nodular goitre. Nodules may be solid, filled with fluid, or partly fluid and partly solid. It's possible for an enlarged thyroid to continue functioning well and producing the right amounts of hormones. In fact, most goitres and nodules don't cause health problems. However, goiter can also be a sign of certain conditions that cause the thyroid to produce too much thyroid hormone (called hyperthyroidism) or too little (called hypothyroidism).

Many goitres and thyroid nodules are harmless, so we often can take a watch-and-wait approach. Additionally, some cases are caused by thyroid cancer, although this is rare. Treatment options include thyroid hormone medication, radioactive iodine therapy and surgery.
